The latest revision to the VHSL Athletic Participation Form is February 2017. Previous versions are not acceptable. Please fill in all blanks and sign where required to avoid a delay in participation. The APS Participation form should be turned in with the VHSL form to the Activities Office in person (preferred), electronically, or by U.S.mail. Concussion Fact sheets should be kept at home for reference.

APS will baseline test student-athletes engaged in contact/limited-contact sports as well as students with a known concussion history. The baseline test is re-administered every two years.
