General Info for Athletics - READ FIRST
Necessary information on eligibility and form requirements for participation in high school sports.
VHSL Physical Form
A new physical exam is required for each school year. Exams must be done after May 1st for the upcoming school year and remain on file for the full year. Be sure to complete entire form by filling in all blanks and signatures. Failure to do so may delay participation.

APS Participation Agreement
Read, sign, and submit with the VHSL Physical Form. Failure to submit the APS Agreement may cause a delay in participation.

No student will participate in any athletic activity without evidence of accident insurance coverage. Affordable Student Accident Insurance is available for purchase for individuals who do not have insurance or wish additional insurance. Concussion ImPACT Test Information
APS will baseline test student-athletes engaged in contact/limited-contact sports as well as students with a known concussion history. The baseline test is re-administered every two years.
